Pay Range: $135004 - $200000/annum. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

• Incentive Bonus: 17.5%
• Sign-On Bonus: $10,000
• Relocation: Case-by-case basis
• Direct and evaluate daily Case Management department operations, staffing, technology, onboarding, and competencies.
• Oversee Case Management core functions, care coordination, and discharge planning processes.
• Ensure timely, high-quality documentation and compliance with organizational policies and regulatory requirements.
• Analyze operational data and develop sustainable performance improvement initiatives.
• Allocate resources effectively to meet productivity and quality goals.
• Build strong relationships with physicians and key stakeholders, including CMO, CNO, ancillary leaders, Ethics & Compliance, and Legal.
• Partner with the Facility CFO and Division Case Management leadership to achieve organizational goals.
• Lead change management, team development, and departmental initiatives.
• Oversee CM, Social Work, and Chaplain services while coordinating efforts across multiple stakeholders.

Key Requirements and Technology Experience:

• Bachelor's Degree – Required
• Master's Degree in Nursing, Health Administration, or Business Administration – Preferred
• Current RN, LCSW, or LMSW licensure – Required
• 3 years of overall acute care hospital Case Management experience – Required
• 2 years of Case Management leadership experience – Required
• Extensive Case Management leadership experience with a proven record of achieving results.
• Strong data analysis, strategic planning, communication, and relationship-building skills.
• Ability to work independently, manage multiple priorities, and develop action plans.
• Strong relationships with physicians and post-acute community partners preferred.

• 790-bed hospital
• Level II Trauma Facility
• 34,000 annual admissions
• Approximately 88,000 annual ED visits
• 215,000 annual outpatient visits
• Approximately 110 FTEs
• Direct reports include 4 Managers, 1 Social Worker Supervisor, and 1 Administrative Assistant
• Oversight of Case Management, Social Work, and Chaplain departments
• Weekend on-call rotation may be required depending on hospital volume and operational needs
