Pay Range: $43 - $48/hour.  Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
 
Key Responsibilities:

• Assessment & Planning-Assess patient medical, psychosocial, and care transition needs.
• Collaborate with physicians and care teams to determine the most appropriate discharge plan and care goals.
• Evaluate community, insurance, and support resources required for ongoing care.
• Care Progression-Monitor patient care progression to ensure timely and efficient treatment and discharge.
• Identify and resolve barriers to discharge, escalating when necessary for resolution.
• Participate in interdisciplinary rounds to align discharge goals and expected timelines.
• Disposition & Post-Acute Coordination-Develop individualized discharge plans in collaboration with patients, families, and the care team.
• Initiate referrals for post-acute services, including home health, rehab, DME, or skilled nursing placement.
• Ensure patient and family understanding of discharge instructions and follow-up care needs.
• Documentation & Communication-Document assessments, plans, interventions, and patient/family interactions accurately in the medical record.
• Communicate effectively with payer sources, utilization review teams, and community resources.
• Track and report avoidable delays or barriers impacting care progression.
• Mentorship & Professional Growth-Support new team members through orientation and mentorship when needed.
• Participate in departmental performance improvement and professional development activities.
• Maintain required competencies and education

Key Requirements and Technology Experience:

• Skills-BSN, BLS, experience as a Registered Nurse.
• Associate Degree in Nursing or Diploma in Nursing required; Bachelor’s Degree in Nursing preferred.
• Active Registered Nurse license (State or Multi-State Compact).
• Basic Life Support (BLS) Certification required.
• ACM or CCM Certification preferred (not required).
• Minimum 3 years of acute care bedside RN experience required.
• Minimum 2 years of hospital-based case management or payer case management experience required.
• Strong communication, critical thinking, problem-solving, and coordination skills.
