Position Summary
The Certified Nursing Assistant (CNA) supports patients on the 4S medical surgical unit, including patients receiving transitional or swing bed care. Under the direction of a licensed nurse, the CNA provides hands-on personal care, helps patients move safely, obtains and records routine measurements, and promptly reports changes or concerns. The night shift CNA helps maintain a safe, calm environment while meeting patient needs throughout the shift.
Essential Responsibilities
Receive assignments and participate in shift report and huddles. Confirm each patient’s mobility, fall, isolation, skin, toileting, and observation needs with the assigned nurse.
Assist with bathing, oral care, dressing, toileting, incontinence care, bed changes, positioning, comfort, and other activities of daily living while preserving dignity and privacy.
Assist with ambulation, transfers, and use of approved lift or mobility equipment according to the care plan and safe patient handling practices. Answer call lights and respond promptly to requests for help.
Obtain and document vital signs, weights, intake and output, and other assigned observations after training and competency validation. Report abnormal results promptly to the licensed nurse; do not interpret results or independently change the plan of care.
Observe and report changes in condition or behavior, including new confusion, breathing difficulty, pain, bleeding, weakness, skin concerns, poor intake, or possible safety risks. Activate immediate assistance for urgent changes according to unit procedure.
Carry out assigned fall prevention, rounding, repositioning, skin protection, and patient observation measures. Follow the nurse’s direction for patients needing enhanced observation and document assigned checks as required.
Assist with meals, snacks, hydration, and collection of ordered specimens within training and hospital policy. Communicate swallowing concerns, low intake, or other difficulties to the nurse.
Prepare rooms and supplies for admission, transfer, and discharge; help maintain clean patient spaces, stocked supplies, and working equipment. Assist with transport as assigned.
Document care and observations accurately and on time in the approved record. Communicate unfinished tasks and patient needs to the nurse and incoming team at handoff.
Use standard and transmission based precautions, hand hygiene, PPE, and safe equipment cleaning. Protect confidential information and follow patient rights, abuse reporting, emergency response, and hospital safety policies.
Work cooperatively with nurses, providers, therapists, and other team members; prioritize assigned tasks as patient needs change during the night.
Qualifications:Current Indiana Certified Nurse Aide status on the Indiana Aides Registry, maintained throughout employment.
Basic Life Support certification at hire or within the hospital’s required onboarding period.
Ability to use an electronic health record and communicate clearly with patients, families, and the care team. Prior hospital, med surg, or transitional care experience preferred.
Completion of hospital orientation and demonstrated competency for assigned tasks and equipment.
