Overview
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Hospice team. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. Responsibilities
- Conduct comprehensive assessments of patients' health status and needs.
- Develop and implement individualized care plans based on patient assessments.
- Administer medications and treatments as prescribed, including IV infusions.
- Provide education to patients and families regarding health management, medications, and discharge planning.
- Document all patient interactions and medical information accurately in medical records.
- Collaborate with interdisciplinary teams to coordinate care for patients with diverse needs, including behavioral health.
- Perform triage for incoming patient inquiries and concerns.
- Offer support in family planning and toddler care as needed.
- Maintain compliance with all regulatory requirements and best practices in home health care.
Experience
- Current Registered Nurse (RN) license in the state of practice.
- Previous experience in home health, ER, or clinic settings is preferred.
- Proficiency in medical documentation practices is essential.
- Experience with medication administration and IV infusion techniques is highly desirable.
- Familiarity with discharge planning processes is beneficial.
- Strong interpersonal skills to effectively communicate with patients, families, and healthcare teams.

Pay: $30.00 - $45.00 per hour
