Position Overview

Putnam County EMS is seeking an experienced, motivated, and compassionate Mobile Integrated Health (MIH) Coordinator to support an established Mobile Integrated Health program serving residents throughout Putnam County.

The MIH Coordinator serves as the primary point of coordination between patients, healthcare providers, community organizations, and EMS personnel. This position is responsible for managing referrals, coordinating patient services, maintaining community partnerships, supporting data collection and reporting, and helping ensure continuity of care for vulnerable and high-risk populations.

The successful candidate must possess strong communication skills, sound professional judgment, excellent organizational abilities, and the ability to work independently while collaborating effectively with healthcare and community partners.

This full-time position is currently funded through the Indiana Family and Social Services Administration (FSSA) Mobile Integrated Response System (MIRS) grant program through 2028, with the potential for future continuation based on program outcomes and funding availability through the Indiana Rural Health Transformation Grant.

Primary Responsibilities


Program Coordination

• Coordinate daily MIH operations, including referral intake, patient scheduling, follow-up activities, and care coordination.

• Maintain active patient caseloads and ensure continuity of care through regular patient contact and follow-up.

• Assist with identifying barriers to care and connecting patients with appropriate community resources.

• Participate in multidisciplinary care planning and case review activities.


Community Partnership Development

• Serve as a liaison between Putnam County EMS and community stakeholders.

• Develop and maintain collaborative relationships with:

o Healthcare providers and hospital systems

o Public health agencies

o Behavioral health providers

o Law enforcement and corrections partners

o Recovery organizations

o Housing and social service agencies

o Community-based organizations

• Represent Putnam County EMS professionally during meetings, case conferences, and community engagements.


Documentation, Reporting, and Quality Improvement

• Complete timely and accurate documentation within ESO and other approved reporting platforms.

• Maintain referral, patient encounter, and outcome tracking records.

• Assist with grant reporting, data collection, quality improvement initiatives, and program evaluation activities.

• Support development of operational reports and performance metrics.

Field-Based Patient Care

• Conduct home visits and community-based patient encounters as appropriate.

• Perform assessments and interventions within the scope of practice of an Indiana licensed paramedic.

• Work collaboratively with healthcare providers and community partners to support patient goals and improve outcomes.


Minimum Qualifications

Required

• Indiana Licensed Paramedic in good standing

• Valid Indiana Driver’s License

• Excellent verbal and written communication skills

• Strong organizational and time-management skills

• Ability to work independently and manage multiple priorities

• Demonstrated ability to build and maintain professional relationships

• Experience working with healthcare, public safety, behavioral health, social service, or community-based organizations

• Proficiency with Microsoft Office applications and electronic documentation systems

Preferred

• Community Paramedicine or Mobile Integrated Health experience

• Case management, care coordination, or healthcare navigation experience

• Experience with ESO electronic health records

• Grant-funded program experience


Certification Requirement

Certification as a Community Paramedic (CP-C) through the International Board of Specialty Certification (IBSC) is required within twelve (12) months of hire. Certification costs will be supported through available grant funding.
