Home Health Licensed Practical Nurse
Servicing: St Joe County (South Bend, Mishawaka, Osceola, and Granger) or Elkhart County (Elkhart and Bristol)
Part Time 4-5 days/wk (20-25 visits/week)

Qualifications and Requirements:
- Current license with the state Board of Nursing
- 1 year of nursing experience is required - home health and OASIS experience preferred
- Ability to perform tasks involving physical activity, which may include heavy lifting up to 50 lbs, bending, and standing
- Current Driver's License, vehicle insurance, and/or access to dependable transportation
- Ability to successfully complete a background check
