Key responsibilities
- Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting
- Assist with mobility, transfers, ambulation
- Prepare meals and assist with feeding as needed
- Support medication self-administration through reminders (no independent med administration)
- Monitor and report patient condition and changes to the supervising nurse
- Take and record basic vital signs when trained and assigned
- Maintain a clean, safe, and orderly client environment
- Assist with linens and light client laundry as appropriate
- Use safety equipment properly (rails, supports, stools, etc.)
- Document care provided according to company and regulatory standards
- Travel reliably to client homes and assigned visits across your service area
- Complete required in-service training, including annual dementia education

Must-have requirements
- Age 18 or older • Active HHA or CNA certification or willingness to obtain certification • Willingness to complete competency and skills assessments • Ability to follow written care instructions and document services • Compassion, patience, and professionalism with vulnerable populations • Ability to work independently in home settings • Reliable transportation and ability to travel to client homes • Ability to lift up to 25 pounds • Willingness to complete TB testing, background screening, and onboarding requirements
