Qualifications and Requirements:
- Minimum 18 years old
- Ensure the schedule is completed in such a way as to retain all clients, hours, and caregivers
- Communicate staffing and availability needs to the appropriate supervisor
- Positive attitude, great communication skills, and ability to work independently
- A solid personal character with an empathetic personality
- Must be able to pass both a criminal background check and drug screen
Qualifications:
Overview:
The Care Transition Coordinator is an experienced field marketer responsible for company growth. As a Care Transition Coordinator, you will be responsible for developing our Home Health/Home Care presence within Supportive Livings and Home and Community-based residences. Responsible for generating new referrals by building relationships with area hospitals, healthcare systems, Case Managers, and other community resources.
Essential Duties and Responsibilities:
- Participate in short and long-range planning to achieve company goals and objectives.
- Responsible for complete referral process, including identifying home health needs, verification of benefits, and coordination of care.
- Cultivate new and existing relationships resulting in company growth.
- Represent the agency in a professional, prepared, and caring manner.
- Prepare weekly reports on all marketing activities.
- Initiate and expand referral network.
- Comply with all aspects of the company's compliance and HIPPA regulations.
- Performs other duties as assigned.
Education and Experience:
- Bachelor's Degree Preferred
- One year of Home Health/Home Care, marketing, or case management experience or a combination of each.
- Must possess a valid driver's license.
- This professional will uphold the policies and procedures established by Purpose Home Health. They must also demonstrate excellent oral, written, and active listening skills.
- Ability to work independently, maintain confidentiality, and meet assigned deadlines.
- Demonstrate sound judgment and good decision-making skills.

- Ability to organize and prioritize work.
