What are the responsibilities and job description for the Outpatient Physical Therapist position at Purpose Physical Therapy?
JOB TYPE: 40 hours/week (Full Time Position)
POSITION SUMMARY:
This role is responsible for providing medically prescribed, goal-oriented physical therapy in a patient’s home or select location based on treatment/patient needs. The role includes providing therapeutic treatments and participating in the development of patient physical therapy plans of care.
PRINCIPAL ACCOUNTABILITIES:
- Perform initial and ongoing physical therapy evaluations and re-evaluations according to APTA standards of practice as well as state standards.
- Develop a plan of care/treatment plan for each patient based on the initial evaluation and objective findings, including short and long-term goals, keeping the patient and caregiver as the focus of care.
- Implement the treatment plan on an established schedule and adjust this schedule and plan according to patient progress.
- Document patient progress according to approved medical documentation and facility documentation procedures.
- Evaluate mobility, strength, and current level of patient function, while setting goals to improve in these areas that are scaled to patient abilities and care realistically attainable.
- Report patient progress to the patient, patient caregivers, and healthcare professionals involved in the ongoing treatment/care of patients.
- Develop a plan for patients following completion of their Plan of Treatment with appropriate support and follow-up services if needed.
- Maintain professional and technical knowledge through attending educational workshops, reviewing professional publications, etc.
QUALIFICATIONS:
- Compassionate and able to deliver quality patient-centered care
- Excellent problem-solving, interpersonal, and communication skills
- Motivated and able be excel in a self-managed environment
- Desire to work a flexible schedule based on patient needs
- Ability to travel to patients' homes or the desired location for treatment
EDUCATION:
- Bachelor’s Degree in Physical Therapy from an accredited physical therapy program.
CERTIFICATIONS, LICENSES, AND REGISTRATIONS:
- Currently-licensed Physical Therapist in the state of Wyoming
- Possesses and maintains a current CPR certification
- Valid driver’s license and auto liability insurance
