Heartline Staffing is recruiting a Family Nurse Practitioner for a full-time outpatient primary care opportunity in South Bend, Indiana. This role focuses on whole-person primary care, health promotion, disease prevention, chronic disease management, patient education, and appropriate referrals within a team-based clinical environment.
The FNP will have clinical support while maintaining meaningful autonomy in day-to-day patient care and serving an underserved patient population.
COMPENSATION
$110,000-$130,000 annual base salary
Student loan repayment eligibility of $50,000-$100,000
Relocation assistance available
SCHEDULE
Monday-Friday
8:00 AM-5:00 PM
No call
No nights
No weekends
POSITION OVERVIEW
Provide comprehensive outpatient primary care.
Support health promotion and disease prevention.
Diagnose and manage chronic health conditions.
Provide patient education and coordinate appropriate referrals.
Deliver whole-person care within a team-based outpatient environment.
Maintain provider autonomy with full clinical support staff.
Meet a minimum of 2,946 outpatient visits annually.
Utilize EHR-supported clinical workflows.

Certified Family Nurse Practitioner REQUIRED
1-3 years of Nurse Practitioner experience REQUIRED
Clear and active Indiana Nurse Practitioner license or ability to obtain one REQUIRED
Prescriptive authority REQUIRED
DEA registration REQUIRED
Current CPR certification REQUIRED
Ability to provide comprehensive, whole-person outpatient primary care REQUIRED
Preferred
Spanish bilingual ability
