Role Description The Licensed Clinical Social Worker (LCSW) at Pure Life Renal will provide psychosocial assessment, counseling, and support services to dialysis patients and their families in an on-site clinical setting located in Leesburg, VA. This part time role includes developing individualized care plans, addressing adjustment to chronic illness, and coordinating resources such as transportation, financial assistance, assisting with insurance issues and community support programs. The LCSW will collaborate closely with nephrologists, nurses, and other interdisciplinary team members to promote holistic, person-centered care. Daily responsibilities include case management, crisis intervention, facilitation of support and education, and documentation in accordance with regulatory and organizational standards. The role also involves advocating for patients’ needs, contributing to quality improvement initiatives, and supporting a therapeutic, respectful environment.
Qualifications
- Demonstrated proficiency in Social Work and Medical Social Work in a healthcare or dialysis setting.
- Experience in Case Management, including care coordination, resource linkage, and health insurance assessment.
- Knowledge and practice experience in Mental Health and Behavioral Health, including counseling and crisis intervention.
- Active Licensed Clinical Social Worker (LCSW) license in the state of Virginia.
- Master’s degree in Social Work (MSW) from an accredited program.
- Strong communication, documentation, and interpersonal skills for collaborative work with patients, families, and interdisciplinary teams.
- Ability to work on-site in a fast-paced clinical environment and manage a diverse caseload.
- Experience in nephrology, dialysis, or chronic disease management is preferred but not required.
