Responsibilities
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner.
- Administer medications and injections accurately following established protocols, including IV infusions and medication management for pain control, hospice care, or chronic conditions.
- Assist with complex clinical procedures such as catheterization, wound suturing, tube feeding, and sterile technique applications in various settings like medical-surgical units or long-term care facilities.
- Monitor patients’ physiological responses during treatments like ventilator management, cardiac catheterization, or spinal taps; respond promptly to acute changes in condition.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans; participate in discharge planning and case management activities.
- Maintain meticulous medical documentation adhering to HIPAA regulations and ICD coding standards (ICD-9/10), ensuring accurate records of patient intake, assessments, treatments, and progress notes.
- Provide education and support to patients and their families on health coaching topics such as dementia care, Alzheimer's disease management, nutrition, or post-acute care strategies.
Experience
- Valid Licensed Practical Nurse license with current state certification required.
- Proven experience working in diverse healthcare environments such as hospitals (including ICU or Level I Trauma Centers), nursing homes, hospice settings, or outpatient clinics.
- Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, eClinicalWorks or Athenahealth for documentation and order entry.
- Critical care experience preferred—especially in ICU or emergency room settings—along with familiarity with acute pain management and infectious disease care.
- Experience working with special populations including individuals with developmental disabilities, dementia or memory care needs is highly valued.
- Knowledge of physiology, anatomy, pharmacology principles related to medication administration and patient assessment is essential.
- Additional experience in specialized areas such as pediatric (PICU), neonatal ventilator management, or home & community care is a plus.
