What you’ll do
- Deliver direct patient care, including administering medications, monitoring vital signs, and assisting with daily living activities.
- Collaborate with healthcare professionals to develop and implement individualized care plans tailored to each patient's needs.
- Conduct thorough patient assessments to identify health issues and report findings to the supervising nurse.
- Maintain accurate medical documentation in compliance with HIPAA regulations and facility policies.
- Provide education and support to patients and their families regarding health management and treatment plans.
- Assist in various medical procedures, including IV insertion, catheter care, and specimen collection.
- Participate in discharge planning to ensure smooth transitions for patients moving from one level of care to another.
- Engage in ongoing professional development to enhance your nursing skills and stay current with best practices.
Basic qualifications
- Valid LPN license in the state of practice.
- Strong knowledge of medical terminology, anatomy, physiology, and pharmacology.
Preferred qualifications
- Experience with electronic medical record (EMR) systems such as Epic or Cerner.
- Background in critical care or acute care settings, including ICU or emergency room experience.
- Familiarity with long-term care, assisted living facilities, or hospice care environments.
- Skills in triage, pain management, and working with patients who have developmental disabilities or dementia-related conditions.
- Proficiency in performing phlebotomy, administering injections, and conducting physical examinations.
