What are the responsibilities and job description for the Speech-Language Pathologist, PRN position at Puller Veterans Care Center?
Purpose of Position: Develop and administer speech therapy treatment programs to resident patients. Prepare clients and deliver treatments, perform SLP evaluations and re-certifications; develop ST treatment and make modifications as needed. Documents client’s status adhering to Federal, State and Facility guidelines.
Clinical Treatment
Establishes skilled speech treatment plans for residents. Conducts skilled speech services to residents once treatment plans are made including but not limited to; symbolic language, dysphasia, aphasia, voice treatment and treatment for communication and assistive devices.
Documentation/Communication
Works cooperatively with other IDT members to design integrated plans and provide supportive documentation. Goals, objectives, and plans are identified for all patients receiving ST services within establiched time frames or of recommended changes and all target dates are current. Ensures assess-ments are completed within established timeframes and that cost information is forwarded to the appropriate sources in a timely manner. Attends and actively participates in clinics and meetings. Uses the appropriate lines of communication and resources within the system to facilitate change and resolve problems.
In-house Activities
Provides direction and training for speech-related services implemented by non-speech staff as assigned or as necessary. Provides in-service training to departmental staff on topics relevant to departmental goals. Attends and successfully completes all mandatory training as scheduled. Assists with additional activities as assigned.
Perform Evaluation and Recertification of residents of S.T. caseload
Review medical records to collect data that is relevant to the assessment as assigned and instructed; Gathers additional assessment information through interview of client, staff, and family as assigned. Completes initial evaluation and subsequent re-certifications as needed.
Ancillary Duties
Attends department and other meetings. Conducts department meetings as assigned and completes ancillary documentation and communication. Follow all safety rules and regulations. Initiates and recommends appropriate corrective action necessary when violations are seen. Maintains clean and safe environments ensuring orderly work area throughout the unit are maintained. Will recommend purchase of new equipment. May serve on committees as designated by supervisor and compete with special project as assigned. Will perform supervision of CFY candidates according to ASHA regulations.
Other Duties
All other duties assigned by the Director of Rehabilitation.
