What You'll Do:
- Assess the psychosocial status of patients
- Provide community and governmental resources
- Conduct social evaluations and plan interventions
- Develop individualized care plans
- Assist physicians with social and emotional factors
- Participate in quality assessment performance improvement
- Liaise between patients and community agencies
- Maintain and develop contracts with public and private agencies
- Assist with bereavement coordination
Qualifications:
- Accredited master’s degree in social work
- Minimum one year of healthcare experience
- Home health or hospice experience preferred
- Current CPR Certification
- Licensed driver with an insured automobile
