Job Overview: Provision of nursing care services and care management for women with children (from birth to 12 years of age) at Interim House West. Responsibilities include a variety of tasks under the direct supervision of the Program Director.

The Registered Nurse is an integral part of the management team. It is responsible for the overall coordination of IHW’s medical department, consisting of one RN, a half-time LPN, nursing assistants, residential assistants, and a program psychiatrist. IHW is an evidence-based/ supported holistic substance use program for pregnant and parenting women with up to 3 children, and is located in the Parkside section of Philadelphia. The program has the capacity to serve 22 women and up to 45 children (ages 0 to 12) at any given time. IHW serves women who suffer from addiction and/or trauma-related disorders; many are referred from the criminal justice system. Primary responsibilities include overall responsibility of the medical department, supervision of LPNs, Nursing assistants, residential assistants, as well as serving as healthcare liaison for our program psychiatrist, coordinating and managing client medical and dental appointments, dispensing medication, including MAT’s, and handling sick call.

Responsibilities:

  • Complete initial resident and children nursing assessments, which include medical history assessments, UDS, and 24-hour diet recall.
  • Perform comprehensive physical / nursing assessments.
  • Provide health teaching, health counseling, and provision of care supportive of restoration of life and well-being to all members.
  • Schedule and/or assist residential medical and dental appointments and ensure Medical Consultations are written so members can present at their medical/dental appointments.
  • Coordinate all members' medical services external to residential programs.
  • Make referrals for clients needing further assessment or management of episodic and/or chronic health problems and ensure follow-up care.
  • Maintain medical records for members.
  • Coordinate members’ medical needs and significant diagnoses and treatment with medical providers.
  • Supervisory and administrative tasks include supervision of LPN, nursing assistants, residential assistants, and participation in weekly team management meetings, handling medical on-call, and transcribing Medication Administration Records (MAR’s) every month.
  • Serve as liaison to the Laboratory for all drug screens and lab work.
  • Ensure all medical-related significant incidents are properly reported to all appropriate agencies, and follow-up investigations are completed.
  • Procure medical supplies needed.
  • Liaison to Medical Director (psychiatrist), including creation of the weekly member schedule and processing of all oral and written orders.
  • On-call responsibility for healthcare-related issues.
  • Obtaining UDS is required.
  • Ability to travel and cover medication management at designated Behavioral Health off-site locations if needed.
  • Valid driver's license required.


Skills:

  • Competent in Medication Administration, reading and interpreting critical lab values, performing UDS, and completing Nursing Assessments
  • Understand and apply setting-specific policies and procedures for handling crisis or dangerous situations, including safety measures for clients and staff.


Experience:

  • 1 year of chemical dependency and other addictions, and appropriate treatment modalities are required. Pediatric experienced a plus.


Education Requirement:

  • Graduate from an accredited nursing school/ university with a minimum of 2 years of pertinent experience.
  • Licensed Registered Nurse
  • Experience in working with diverse populations and knowledge of diverse cultural needs.
  • Basic understanding of medical and psychosocial causes relevant to chemically dependent persons.
  • Ability to work with and educate non-medical staff.
  • Knowledge of chemical dependency and the recovery process is required.
