The Case Manager/Parent Educator will devote time to both roles. This position works with the Focus on Fathers team to provide case management services to adult fathers enrolled in the program. This person maintains weekly contact with participants through meetings (in-person or virtual) or phone calls. This person will be responsible for intake assessments, identifying and setting goals with participants, making appropriate referrals for supportive services, and completing periodic progress assessments, including closing assessments, to document participant progress. This person may also make warm transfers for services and/or accompany participants for social services and medical appointments, as appropriate. In the capacity of Parent Educator, the employee provides parenting education sessions to fathers, couples, co-parents and other caretakers of children—reports to Program Manager.

Case Management Responsibilities:

  • Carries an ongoing caseload of 15-25 fathers
  • Assesses participant needs and strengths with input from other team members (i.e., parent educator, program manager)
  • Administers evaluation instruments, including intake and closing assessments, and maintains contact notes aligning with program and evaluation requirements
  • Develops service plans that support both program and participant goals
  • Utilizes a variety of contact methods to reach participants
  • Provides referrals and linkages for participant to services, including custody and child support case support, trauma counseling, mental health or substance abuse treatment, domestic violence treatment, and various other needs
  • Meets participants in a variety of settings, including online (virtually), participant home, and community
  • Actively supports and facilitates the ongoing recruitment and retention of parent graduates (fathers) to participate in the Focus on Fathers Alumni Parent groups
  • Participates in case conferences and trainings, and weekly team meetings
  • Actively engages in evaluation activity by attending/contributing to CQI team meetings
  • Works as part of a team; can function in peer-to-peer learning environment
  • Works with the program team to assist in the delivery of other program activities
  • Assists with program evaluation by entering participant data via web-based application
  • Complete program reports and other written documentation as required


Parent Education Responsibilities:

  • Provide on-going parenting skills education groups for fathers, couples and co-parents at various community-based sites, using established curricula
  • Assist with tracking of participant progress to ensure course completion
  • Assist in identifying, recruiting, and engaging clients for the program
  • Complete program reports and other written documentation as required
  • Works closely with partner sites to ensure participant retention through follow up calls and in-person engagements
  • Work collaboratively between other programs in HPC and other agencies/organizations
  • All other responsibilities as assigned.


Skills:

  • Excellent oral communication skills
  • Ability to engage underserved and hard-to-reach populations
  • Strong organizational skills, including the ability to prioritize multiple assignments
  • Knowledge of issues related to child development, family life skills, child abuse prevention, healthy relationships, and healthy parenting
  • Availability during daytime and some evening and weekend hours
  • Willingness to travel throughout Philadelphia


Experience:

  • Two years direct service experience working with men and families
  • Experience with clients facing substance abuse, mental health, employment, criminal justice and other issues
  • Strong experience with adult education and/or group facilitation
  • Comfortable working with adults in low-income communities
  • Knowledge of positive parenting principles and practices
  • Personal commitment to promoting and being a role model for fathers, couples/co-parents
  • Must be willing to work flexible schedule including some evening/weekend hours as requested
  • Must successfully pass child abuse clearance
  • Proficient with Microsoft Office, and web-based data collection applications


Education Requirement:

  • Bachelor’s degree in public health, social work, or related field preferred; or equal relevant work experience.
