Position Summary

The Case Manager provides direct support to veterans participating in Camp Hope's interim housing program by helping them access resources, overcome barriers, and work toward greater independence. The Case Manager serves as a primary point of contact for residents, assisting with goal setting, connecting them to community resources, monitoring progress, and coordinating services throughout their stay.

This is an entry-level professional position designed for individuals who are passionate about serving veterans and helping people navigate complex systems. Training program procedures and available resources will be provided.

Primary Responsibilities

· Meet regularly with assigned residents to identify needs and monitor progress.

· Assist residents in developing and working toward individualized goals.

· Connect residents with community resources, including housing, employment, education, transportation, legal services, healthcare, and public benefits.

· Assist residents with obtaining identification documents, completing applications, and accessing VA and community resources.

· Coordinate appointments and referrals with internal staff and external community partners.

· Maintain regular contact with residents and encourage continued engagement in the program.

· Document all client contacts, referrals, and progress in the electronic record system.

· Participate in team meetings and collaborate with program staff to support resident success.

· Monitor follow-up tasks and ensure services are completed in a timely manner.

· Maintain confidentiality and comply with organizational policies.

· Perform other duties as assigned.

Minimum Qualifications

· Bachelor's degree preferred but not required.

· Bachelor of Social Work (BSW) preferred but not required.

· Master of Social Work (MSW) preferred but not required.

· Degrees in social work, psychology, sociology, criminal justice, human services, public health, counseling, or related fields are encouraged to apply.

· Relevant experience working with veterans, nonprofit organizations, customer service, community outreach, or human services is preferred but not required.

· Strong customer service and communication skills.

· Experience working with diverse populations.

· Ability to build positive working relationships with residents and community partners.

· Strong organizational and time management skills.

· Excellent verbal and written communication.

· Ability to manage multiple priorities.

· Basic computer proficiency, including Microsoft Office and electronic documentation systems.

· Ability to work independently while collaborating with a team.

· Professionalism, reliability, and compassion when working with residents.

Work Environment

This position is based in an Interim Housing program. The Case Manager spends time meeting with residents, coordinating services, documenting activities, and working with community partners. Some local travel may be required for meetings or resource coordination.

Success in this position is measured by:

· Timely completion of assigned tasks and documentation.

· Consistent engagement with assigned residents.

· Effective coordination of referrals and community resources.

· Accurate record keeping.

· Professional communication with residents, staff, and community partners.

· Positive contributions to the overall success of the program.

Compensation and Benefits

· Salary: $50,000–$60,000 annually

· Full-time benefits package, including health insurance, paid time off, retirement benefits (if eligible), and professional development opportunities.
