1. Demonstrates Skillful Performance in the Practice of Physical Therapy.

a. Completes an appropriate patient assessment and evaluation to include the patient's functional status and type of care provided. The evaluation could include, as appropriate to the scope of care provided by the professional, but is not limited to: the level of independence in appropriate environments, mobility status, problems with continence, the ability to operate and maintain equipment, communication skills, memory, cognitive level, orientation, emotional response to current health status, pain, and dental function. The functional assessment could include, as appropriate to the scope of care provided by the professional, but is not limited to: range of motion, muscle strength, the incision site, gait on flat surfaces and stairs, bed mobility, transfers in various settings, housekeeping, and home maintenance.

b. Regularly conducts physician prescribed treatments in a conscientious manner.

c. Thoroughly explains procedures and course of treatment to patients; allows time for patient questions.

d. Verifies identification of patient prior to procedure.

e. Thoroughly reviews documentation or patient chart prior to treatment.

f. Completes documentation in a neat and accurate manner.

g. Ensures equipment is in proper working condition and is well supplied prior to beginning treatment.

h. Demonstrates rapport with patients and family members during instruction of home therapy programs.

i. Observes patient during treatment and therapy to ensure patient safety. Identifies and follows safety procedures and medical precautions.

2. Demonstrates Sound Judgment and Decision Making Skills.

a. Always attempts to understand patients needs, and responds accordingly.

b. Demonstrates sound judgment, based on principles of patient therapy.

c. Seeks guidance and direction as necessary.

d. Reports changes in patient's condition or behavior to appropriate person.

e. Demonstrates ability to assess patients and choose appropriate course of action.

f. Recognizes role of physical therapy in the department and how it relates to the overall function of the clinical setting.

g. Consistently combines ethical judgment with technical skills and functions within the legal limitations of the profession.

3. Knowledgeable About the Practice of Physical Therapy

a. Demonstrates a complete understanding of policies and procedures concerning physical therapy.

b. Receive and maintain current licensure with appropriate state Physical Therapy Board.

c. Keeps abreast of current trends affecting physical therapy by reviewing professional journals and attending seminars or workshops pertinent to area of practice.

d. Demonstrates complete understanding of the musculoskeletal and neurological systems.

e. Demonstrates a desire to increase knowledge base of physical therapy.

f. Receive and maintain CPR Certification.

4. Functions in an Effective Working Relationship with Supervisors, Co Workers, Patients and Colleagues.

a. Greets patients in a courteous manner; responds to patients with empathy and positive interpersonal skills.

b. Demonstrates good rapport and cooperative working relationships with all members of the Physical Therapy Department and interdisciplinary health team.

c. Demonstrates positive communication skills in all aspects of professional relationships.

d. Respects the confidentiality of all professional interactions.

5. Plans and Implements Time Effectively.

a. Coordinates work to achieve maximum productivity and efficiency.

b. Able to adjust and revise schedule with a minimum of difficulty in order to accommodate changing patient and departmental needs.

c. Demonstrates responsibility in completion of all assigned tasks. Completes paperwork in a timely manner according to agency and department guidelines.

d. Able to recognize and prioritize work responsibilities.

6. Functions Effectively in an Independent Manner; Demonstrates Initiative in Job Performance.

a. Performs duties in an independent manner with minimal or no direct supervision.

b. Recognizes and performs additional duties not directly assigned; assists others with duties in a cooperative manner.

c. Assumes responsibility for on going continuing education; maintains documentation of professional development.

d. Maintains equipment in working order; documents equipment safety checks and maintenance according to department protocol.

e. Offers suggestions for positive changes or recommendations within the department or in reference to the company.

7. Demonstrates Reliability in Meeting Job Expectations.

a. Demonstrates reliability in attendance patterns.

b. Provides proper notification for scheduled absences or illness time off.

c. Consistently is on time and ready to work at beginning of shift.

d. Is physically capable of performing all work related duties.

e. Maintains a professional appearance; adheres to agency dress code policy.

f. Practices infection control in everyday routines.

g. Does not interrupt work schedule for personal matters.

8. Functions in a Productive Manner

a. Completes routine patient treatment procedures in the required time frame.

b. Completes patient treatments scheduled for the day without sacrificing the quality of work performed.

c. Utilizes time effectively, to maximize the productivity of the department and corporation.

d. Willingly accepts work assignment additions.

9. Perform all activities related to clinical and on-site industrial rehabilitation services not limited to functional capacity evaluations, job site analysis, and ergonomic assessments/consulting.

P.T. Services began in 1972 with a small group of physical therapists and has grown into a Tiffin-based company with over 275 employees and numerous locations. When P.T. Services was first incorporated in 1972, its six therapists were practicing at outpatient clinics in Tiffin and Findlay, and at several nursing homes. A corporate headquarters for the company was opened in Tiffin in 1978, where management and support staff provided day-to-day business operations. In 1989, P.T. Services moved its corporate office to its current location at 2550 S. SR 100, Tiffin. In 2011, P.T. Services made an innovative change from being a privately held company to operating as an ESOP corporation. Essentially, an ESOP is a trust with assets that are divided into individual accounts, and enables employees to share in the wealth they share in creating.
