Must possess a high school diploma and be enrolled in a bachelor of social work program, as well as meet the criteria for a Children’s Mental Health Specialist, Qualified Intellectual Disabilities Professional and Qualified Mental Health Professional. Person must have police and DHS clearance. Position Summary
The Supports Coordinator Assistant is responsible for ensuring all service needs of a consumer are met by providing linking, coordinating, advocacy, negotiating and referral services under the supervision/guidance of a Case Manager. The Supports Coordinator Assistant is responsible for the assurance of a Person Centered Plan as well as the monitoring of the established goals and the implementation of various treatment plans in conjunction with the multi-disciplinary team and under the guidance of the Case Manager. The Supports Coordinator Assistant will act as an advocate for the consumers and community served and ensure delivery of services.
Specific Responsibilities
- Develops an individualized plan of service based on the comprehensive assessment which includes goals established by all appropriate disciplines
- Conducts self audits as well as peer audits
- Must be knowledgeable in the Mental Health Code and Medicaid guidelines as well as billing protocols and procedures
- Ensure delivery and monitoring of services as prescribed in the Person Centered Plan
- Maintain PsyGenics’ expected productivity level
- Completes and secures required signatures
- Assists Case Manager in the delivery of referrals, services and implementation of the Person-Centered Plan
- Coordinates and completes all necessary referrals on behalf of client
- Monitor all services provided on behalf of client
- Provide resource development in an attempt to reduce the amount of service gaps
- Develop and maintain written records necessary for professional practice as defined in the procedural manual
- Provide outreach services to individuals and their families in their respective locales and/or homes as appropriate to their unit’s function
- Attend MCCMH and/orDWIHN meetings and trainings as required
- Provide community education services as assigned
- Attend court proceedings as required
- Participate in clinical supervision if necessary, team meetings and case consultations
- Obtain all prior authorizations and applicable coordination of care
- Follow PsyGenics, Inc. professional code of conduct
- Represent PsyGenics, Inc. in the community
- Perform similar or related duties as assigned
This is a full-time position. Salary commensurate with experience.
