What are the responsibilities and job description for the Street Outreach Coordinator position at PsychPros Staffing & Executive Search?
PsychPros is currently partnering with a non-profit in Morehead, KY seeking a Street Outreach Coordinator (Case Manager). In this role, you will work with a team of dedicated social service professionals to help identify and provide outreach services to the homeless population in the community, on the streets and in homeless camps and help connect affected individuals with emergency shelter, housing or other critical services. This position also provides transportation to participants/families as needed.

- Work with other outreach community members
- Identify and engage needed partnerships that will improve services and reduce barriers
- Represents the agency at outside meetings as needed
- Maintains positive relationships and serves as a liaison with other related community service agencies.
- Maintains up to date participation list
- Keeps accurate Data on participants
- Provides information to the supervisor for quarterly and monthly reports
- High School Diploma/equivalent required
- Associates Preferred
- 1-3 years of social service experience
- Ability to work independently
- Ability to develop community resources
- Valid Drivers License and reliable transportation
