We are seeking a dedicated and compassionate Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our rapidly growing team serving long-term care facilities in the Cleveland, Akron, and Canton, Ohio areas. Key Responsibilities:
- Provide psychiatric evaluations, medication management, and ongoing treatment to residents in long-term care facilities.
- Collaborate with interdisciplinary teams to develop, implement, and evaluate individualized treatment plans.
- Maintain accurate and timely documentation in compliance with regulatory standards.
- Demonstrate a high level of integrity and clinical excellence in all aspects of care delivery.
Minimum Requirements:
- Active and unrestricted Psychiatric Mental Health Nurse Practitioner license in the state of Ohio.
- Board Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP).
- Active DEA license.
- Minimum of one year of psychiatric experience required; geriatric experience preferred.
- Strong interpersonal and communication skills.
- Must be willing and able to travel locally up to 1 to 1.5 hours to assigned facilities.
