Program Coordinator - Club 194
Foster a supportive rehabilitation environment by effectively training, supervising employees, overseeing performance, and assessing outcomes. Coordinate programs and services for individuals with intellectual/developmental disabilities in collaboration with agencies, residential service providers, and employees, ensuring compliance with Title 17, Title 22, and CVRC standards. Maintain accurate and up-to-date records, reports, and files for individuals and facilitate clear, timely communication with all relevant parties, both verbally and in writing.
Demonstrate strong judgment, initiative, organizational abilities and leadership in providing guidance and direction to others.
A bachelor’s degree from an accredited college or university is required with relevant work experience may be considered in place of formal education at a two-to-one ratio. Candidates need at least four (4) years of experience in habilitation, social services or education, plus one (1) year in a management role. An accredited “Certificate of Supervision” covering fundamental principles and practices of management and supervision may replace One (1) year of management experience.
Additional requirements: ability to lift up to 50 lbs., DOJ & FBI Live scan background with clearance, and passing a pre-employment physical/TB drug/alcohol screening. Applicants must possess an accredited High school diploma/ GED, be least 21 years of age, clean DMV record, and a current Driver’s License (prefer commercial license).
- Implement admission, assessment, in-house placement and make recommendations to Program Supervisor III regarding referral of individuals assigned.
- Coordinate and implement training and instructions with appropriate agencies or individuals served. Attend IPP (Individual Program Plan) meetings for individuals served as a representative of PSW.
- Ensure all files for individuals served are current with updated, accurate information. Documentation needs to be uploaded into appropriate files in Case Magic. Prepare internal and external reports, including special incidents reports and records promptly and in a professional manner.
- Develop methods, materials and activities that result in growth and reflect current and acceptable habilitation practices for the individuals served.
- Develop, implement and maintain individual program plans discussed with the Interdisciplinary Team. Oversee that documentation of progress is tracked daily.
- Be a resource for employees in the areas of evaluation, assessment, progress, behavioral adjustment, training and related materials in regard to individuals served.
- Assign work schedules and training to staff. Review employees’ time clock information for accuracy. Make adjustments as needed.
- Complete performance evaluations for staff, as assigned. This includes developing goals and working together to achieve a positive outcome.
- Implement disciplinary proceedings, as needed.
- Willing to work directly on the program floor and assist individuals served with personal needs.
- Ensure that individuals served are properly and safely loaded/unloaded on PSW vehicles.
- Provide assistance and/or information to the Program Supervisor III or designee on a regular basis.
- Follow established PSW policies and procedures.
- Perform all other duties as assigned.
