RESIDENT CARE COORDINATOR - FULL TIME - MONDAY-FRIDAY - 8:00am-4:30pm
Job Purpose
Ensures the quality care of all residents. Works closely with the Administrator, Health Services Director, Dining Services Director, Life Enrichment Coordinator, Housekeeping and Environmental Services Director. This position requires creativity and flexibility with residents who have complex situations and is at times stressful. Must train resident aides in tasks required for delivering quality care to the residents of the community.
Key Responsibilities
High school graduate or equivalent
Minimum Experience Required
Minimum of four (4) years’ experience in long term or assisted living elder care. A minimum of two (2) years’ experience in a supervisory role within the healthcare industry, with scheduling experience a necessity.
Minimum Licensure/Certification Required By Law
Current and unexpired CPR certification
ADDITIONAL QUALIFICATIONS: (Preferred qualifications)
Previous supervisory experience in long term or assisted living elder care preferred. Degree in nursing from accredited school of nursing preferred

Job Purpose
Ensures the quality care of all residents. Key Responsibilities
- Coordinates care plan conferences with each resident and responsible family members, sending letters out on a monthly basis, and then following up with resident’s family members.
- Maintains current tasks lists for resident aides, and update timely as ADL changes occur.
- Schedules the service plan dates according to state regulation guidelines.
- Familiar with the service plan program and all of the reports it creates
- Implements interim service plans as care needs change and ensures all medication techs are properly trained on implementing interim service plans
- Monitors the medication room to ensure proper stock, organization and cleanliness.
- Department audits as directed by Health Services Director/Administrator.
- Assists Health Services Director with tracking and recording incident reports and ensures incident investigations are completed timely and thoroughly.
- In collaboration with the Administrator/Health Services Director/Partner Services Manager, hires, trains, supervises, counsels and terminates staff as needed
- Fills-in as needed and directed by the Administrator for receptionist, medication tech, and/or resident aide
