Responsibilities:
- Perform Thorough Evaluations, Assessments, and Treatments for Patients Recovering from Illness, Injury, or Surgery
- Develop and Implement Individualized Care Plans and Therapeutic Interventions to Improve Patient Mobility and Function
- Collaborate Closely with Physicians, Nurses, and Other Rehabilitation Team Members
- Educate Patients and Families on Therapy Plans, Mobility Techniques, and Home Exercise Programs
- Accurately Complete All Required Documentation and Patient Records in a Timely Manner
- Ensure Care and Services Meet Regulatory, Facility, and Professional Standards
- Participate in Quality Improvement Initiatives and Staff In-Services
Qualifications:
- Active, Current, Unrestricted Physical Therapist (PT) License in the State of Practice
- Graduate of an Accredited Physical Therapy Program (CAPTE Accredited)
- Experience in Skilled Nursing, Rehabilitation, or Post-Acute Care Preferred
- Strong Clinical, Communication, and Organizational Skills
- Commitment to Patient-Centered, Compassionate Care
- This Role Is Available for Candidates Who Are Currently on an H1B Visa and Are Eligible for an H1B Transfer
