Physician Assistant or Nurse Practitioner Palliative Care

St. Johns Medical Foundation is seeking a physician assistant or nurse practitioner to join a care team serving homebound patients throughout Santa Monica in California. You will be providing home-based care primarily to homebound patients, while also providing care for Palliative Patients. Ideal candidates will have experience in Palliative Care, Hospice, Pain Management, or Family Medicine, but new graduates with relevant experience are welcome to apply. This role offers both autonomy and collaboration within a compassionate team. You'll be seeing primarily a Geriatric patient population while traveling across the region as a solo practitioner. This is a position that has a huge impact on the overall health and wellbeing of a vulnerable and underserved patient population, and therefore the ideal candidate will be compassionate about the patients they serve. Make a meaningful impact in healthcare while enjoying the scenic beauty of Santa Monica, California. * Full time position * Experienced preferred, Open to New grads * Outpatient care * Monday to Friday schedule * Compensation is between $159,533 and $175,000 per year * The posted salary reflects the starting range of total compensation and may include productivity bonuses, incentives related to quality and performance, hospital/ED call stipends, extra shift incentives, and other forms of cash compensation as applicable to the position. In addition, providers typically receive a CME allowance and other benefits offered by their medical group employer.
