- Plan, administer, and adjust skilled physical therapy treatments to improve patients' physical function.
- Screen and test patients to evaluate strengths, aiding physicians in developing treatment plans.
- Develop and implement rehabilitation programs aligned with physician’s orders and patient goals.
- Collaborate with physicians and clinical teams to refine therapy plans.
- Supervise and evaluate Physical Therapy Assistants and home health staff, ensuring proper documentation.
- Document patient progress, interventions, and evaluations accurately and promptly.
- Communicate patient progress to physicians, families, and care teams, submitting required reports.
- Degree from an accredited Physical Therapy Program.
- Current, unrestricted Physical Therapy license.
- CPR certification.
- Strong organizational and communication skills.
- Valid driver’s license, auto insurance, and reliable transportation.
- Preferred: minimum one year of experience.
