We are seeking a Nurse Practitioner, Advanced Practice Provider to deliver effective, equitable primary care to adult Medicare patients in an outpatient setting.
Key responsibilities include conducting office visits for routine and acute issues, performing wellness visits and health assessments, coordinating care with specialists and testing facilities, leading population health initiatives, educating patients, assisting with phone triage and outreach, and conducting home visits as needed.
The role involves working within a collaborative care team alongside physicians, nurses, and support staff, focusing on primary care, care coordination, and population health management.
Qualifications include a Master of Science in Nursing or DNP, national certification in family, adult-gerontology, or gerontological primary care nursing, an active state NP license, a DEA license, and US work authorization.
Preferred skills are experience in primary care, understanding of HCC documentation, ICD-10 coding, and passion for teamwork and population health. Bilingual abilities are a plus.
