The Coordinator, MDS is responsible for overseeing resident assessments in nursing facilities, ensuring accurate and timely completion of the RAI and other assessment tools in compliance with regulatory standards.
Key duties include collaborating with the Interdisciplinary Care Plan Team to develop comprehensive care plans, assigning assessment reference dates, and verifying that assessments are signed, dated, and completed within required timeframes. They also manage admission/discharge records, certify assessment completion, and ensure adherence to policies and regulations.
Additional responsibilities involve conducting resident/staff/family interviews, maintaining accurate reports, coordinating with management to meet facility needs, and assisting with resident care when necessary. The role may include interim supervisory duties.
Qualifications include a current Registered Nurse license, RAC-CT certification (or within one year), CPR certification, and 2-3 years of healthcare facility experience, preferably in long-term care. Knowledge of clinical software and strong communication skills are essential.
