Full-time Physical Therapist (PT) needed for home health care in the Sebring FL area, Monday to Friday.
Role Overview
Responsible for evaluating patients' mobility, strength, and function through assessments, and developing personalized care plans to help patients recover at home.
Deliver physical therapy services aligned with physician orders, document progress, modify treatment goals, and determine discharge plans.
Collaborate with healthcare team members, including physicians and caregivers, to ensure coordinated care.
Supervise clinical personnel such as PT Assistants and Home Health Aides where applicable.
Complete OASIS assessments as permitted by state regulations, ensuring compliance with policies, standards, and laws.
Maintain effective communication with staff, families, and other healthcare providers.

Qualifications & Skills
- Degree in Physical Therapy from an accredited program.
- Current, unrestricted state license as a Physical Therapist.
- Valid CPR certification.
- Preferred: strong organizational, communication, time management skills, attention to detail, relationship-building experience, and familiarity with home health therapy and OASIS documentation.
