Responsibilities: Conducts psychosocial and functional assessments, collaborates with care teams, develops individualized discharge plans, and coordinates post-acute referrals. Manages barriers to discharge, ensures timely progression of care, and documents interactions accurately. Participates in interdisciplinary rounds and patient education to support seamless care transitions.
Qualifications & Skills: Requires an RN license (single or multi-state), BLS certification, minimum of 1-year acute care nursing experience, and strong communication, assessment, organizational, and problem-solving skills. Knowledge of healthcare regulations and community resources is essential. Demonstrates professionalism, self-confidence, and the ability to work under stress in a fast-paced environment.
Additional Duties: Supports departmental goals, mentors students, and adheres to organizational policies.
