Social Worker MSW Per Diem Variable

**Description** The Social Worker provides comprehensive services to include psycho-social assessments, community resource information and referrals to patients, their families and loved ones, ranging in age from infants through geriatrics, as it supports the continuum of care. Provides coordination of complex discharge planning needs, and facilitates multidisciplinary care team involvement to expedite plan of care for patients. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
