Occupational Therapist - PACE

**Description** This is a full time position supporting our Tanasbourne center and Washington County catchment. This position is responsible for OT evaluations, and development and implementation of the OT plan of care in accordance with PACE policies. The OT is a member of the Interdisciplinary Team and responsible to communicate and coordinate care with the team as well as multiple outside providers, caregivers and family members.
