Intake Assessment Representative - Patient Access

**Description** The Intake Specialist serves as the main access point for Home Health Services. This position is responsible for initiating and coordinating Home Health referrals, gathering all necessary referral and payor information and then inputting into EMR, verifying patient information and initial HH eligibility with patient and referral source, and coordinating and scheduling date for HH admission.
