Intake Assessment Representative - Home Health

**Description** The Intake Specialist serves as the main access point for Home Health Services. This position is responsible for initiating and coordinating Home Health referrals, gathering all necessary referral and payor information and then inputting into EMR, verifying patient information and initial HH eligibility with patient and referral source, and coordinating and scheduling date for HH admission. This position ensures that information obtained at time of referral is accurate and complete. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
