**Description** The Hospice Access Field and Triage RN will complete medical requirements while coordinating a whole-person plan of care. Care is according to the individualized needs of the patient, following the nursing process and in accordance with the Nurse Practice Act. In Field role, the RN will visit patients in their homes (private or facility) to present the hospice program and conduct eligibility assessments, coordinating care with Hospice MD, family and facility caregivers, and interdisciplinary team (IDT). Field RNs will also make urgent and routine home visits to patients already on hospice service. In Triage role, RNs will take patient calls from Halsey Building in NE Portland. The Triage Department is the main access point for all Providence Hospice customers (to include, but not limited to: current patients, current patients' family members, physicians, physician office staff, other service providers, inquiries, etc.). This position will process triage calls form existing patients and caregivers, addressing clinical issues with advice and forwarding to individual team members and managers as necessary. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
