**Description** **Care Manager RN at Providence Medford Medical Center in Medford, OR.** **0.4 FTE available. This position includes working on some weekends as needed.** The RN Case Manager is an expert professional registered nurse who is responsible for developing and managing patient care outcomes for an entire patient case load. The RN Case Manager may perform assessment, treatment or care for patients of all ages, including neonate, pediatric, adolescent, adult and geriatric, based on population focus.
