Seeking a licensed PT or OT with experience or a strong interest in lymphedema management to join an in-home chronic disease management program. Clinicians will provide patient-centered care focused on improving mobility, function, and quality of life.
Key Responsibilities
- Evaluate and treat patients with lymphedema or lymphatic dysfunction.
- Develop and implement individualized treatment plans.
- Perform manual lymphatic drainage, compression therapy, and exercise-based interventions.
- Supervise assistants (PTAs/COTAs) as applicable.
- Maintain timely documentation and collaborate with interdisciplinary teams.
- Licensed PT or OT in Florida (required)
- Lymphedema Certification (CLT - required)
- Minimum 1 year of home health and lymphedema experience (preferred)
- Current CPR certification and valid driver's license
- Bilingual preferred
- Home-based visits (in-home lymphedema program)
- Assigned to a primary zone within the county and a nearby secondary zone to minimize travel
