Position Overview

We are seeking a passionate and dedicated Occupational Therapist (OT) to join a growing rehabilitation team in a Skilled Nursing Facility setting. This is an excellent opportunity to work in a supportive and collaborative environment focused on delivering high-quality, resident-centered care to the geriatric population.

Key Responsibilities

• Evaluate residents and develop individualized treatment plans
• Implement evidence-based occupational therapy interventions
• Monitor patient progress and modify treatment plans as needed
• Complete accurate and timely clinical documentation
• Supervise Occupational Therapy Assistants (OTAs)
• Participate in interdisciplinary care planning meetings
• Collaborate with nursing, therapy, and medical teams
• Support quality improvement initiatives and resident-centered programs
  
  

Qualifications

• Graduate of an accredited Occupational Therapy program
• Active New York State Occupational Therapist License required
• Long-term care or rehabilitation experience preferred
• Strong communication, organizational, and clinical skills
  
  

Schedule

• Day Shift
• No on-call requirements
• No weekend or night shift requirements
