Key Responsibilities
- Conduct initial assessments, evaluations, diagnosis, treatment follow-up, and client education.
- Provide individual, group, and family therapy.
- Coordinate referrals, support services, prior authorizations, and other patient resources.
- Monitor and document changes in treatment, symptoms, interventions, and outcomes.
- Collaborate with psychiatrists, the Medical Director, CEO, and other healthcare providers.
- Serve as an effective communication link between patients and providers.
- Advocate for client safety, quality of care, and overall well-being.
- Provide education to clients and families regarding diagnoses and treatment.
- Assist providers with information relevant to medication management.
- Maintain accurate and confidential patient documentation.
- Prepare monthly productivity and performance reports.
- Participate in required meetings, committees, and in-service training.
- Help identify opportunities to improve departmental efficiency and patient care.
- Maintain current referral and community resource lists.
- Utilize technology and available systems to improve workflow and service delivery.
- Perform other duties as assigned by the supervisor or Medical Director.
- Master's Degree in Social Work (MSW) from an accredited program.
- Active Licensed Clinical Social Worker (LCSW) license.
- 3-5 years of post-master's clinical experience preferred.
- Experience working with children, adolescents, families, and adults.
- Strong knowledge of clinical assessment, treatment planning, documentation, and patient advocacy.
- Excellent organizational, communication, and interpersonal skills.
- Ability to work independently, prioritize responsibilities, and meet deadlines.
- Proficiency with computers, electronic systems, and standard software applications.
- Strong commitment to patient confidentiality, ethics, and professional standards.
