Key Responsibilities
- Conduct comprehensive health assessments, physical examinations, and medication reviews.
- Diagnose and manage acute and chronic medical conditions.
- Develop individualized treatment and care plans in collaboration with the interdisciplinary team.
- Order, review, and interpret laboratory and diagnostic tests.
- Prescribe medications within the scope of practice.
- Master's degree from an accredited Nurse Practitioner program.
- Active New York State Nurse Practitioner or Physician license.
- Active DEA registration.
- Currently enrolled and credentialed with both Medicare and Medicaid.
- Minimum 3,600 clinical practice hours for NPs, in accordance with New York State requirements.
- Minimum 3 years of clinical experience.
- Primary care experience.
- Ability to practice independently.
- Strong communication, clinical, and patient-care skills.
- Part-time: 2 days per week
- Approximately 4-hour shifts
- Monday-Friday
- Preferred shifts: 9 AM-1 PM or 1 PM-5 PM
- On-site position
- No nights
