Job Summary

We are seeking a Family Nurse Practitioner to join a growing Adult Medicine team in Brooklyn, NY. The provider will deliver comprehensive primary care services to adult patients, including preventive care, health assessments, diagnosis, treatment, and chronic disease management in an outpatient setting.

Responsibilities

• Conduct comprehensive health assessments and physical examinations
• Diagnose and treat acute and chronic medical conditions
• Provide preventive care, screenings, and patient education
• Develop and implement treatment plans
• Prescribe medications and order diagnostic tests as appropriate
• Review and interpret laboratory and diagnostic results
• Coordinate referrals to specialists and other healthcare providers
• Maintain accurate and timely patient documentation
• Collaborate with physicians, nurses, and other healthcare professionals
• Participate in quality improvement and patient care initiatives
  
  

Qualifications

• Licensed Nurse Practitioner in New York State
• Family Nurse Practitioner (FNP) certification required
• Minimum of 2 years of experience as a Family or Adult Medicine Nurse Practitioner
• Strong clinical, diagnostic, and communication skills
• Ability to work effectively in a team-based environment
