The Substance Use Disorder Professional/ Trainee (SUD P/T) will conduct case management duties for each patient on his or her caseload, under the guidance their designated supervisor.

Position Summary:
The Substance Use Disorder Professional/ Trainee (SUD P/T) will conduct case management duties for each patient on his or her caseload, under the guidance their designated supervisor.
Essential Job Functions:
- The SUDP/T will conduct chemical dependency assessments and intakes documenting both DSM 5 and ASAM PPC2.
- As well, they will use ASAM PPC2 for continuing treatment review and discharge.
- The SUDP/T will perform individual, group, and family sessions. The SUDP/T will be trained and supervised in conducting sessions by their designated supervisor.
- The SUDP/T will work with their supervisor on a plan of action to meet the requirements described in WAC 246-811-047.
- Case management includes but is not limited to:
- Charting
- Treatment planning & treatment revisions
- Monthly status reporting
- Correspondence
- Online File maintenance to include – review & updating any/all Reporting agents; Consent forms; brief overview of Patient demographics page; Dates of Service attended using Activity 2 report.
- Consult with other clinical staff as needed for the patient’s treatment
- Working with Admin/PC Staff to schedule clients; set up phase graduation/discharge review; update treatment financials and any missing dates of services.
- Must have an active SUDP/SUDPT credential--with no enforcements.
- No experience necessary, will train.
- Must have a High School Diploma or GED
- Dual MH/SUDP credential a plus—can provide LMHCA/LICSWA hours.
- Interest in, or current knowledge of TMS treatment for depression
- Have no prohibition from contracting with governmental agencies.
