Position Description: Community Care Provider

These positions are responsible for providing active treatment (primarily for Activities of Daily Living), implementing individual treatment and behavioral programs, performing general household work, and assisting individuals with intellectual disabilities with normal day-to-day activities. In addition, the individuals served may have mental health or behavioral disorders. These positions assist individuals with acquiring skills for personal independence. Some clients may have physical disabilities requiring specialized equipment and/or manual transfer techniques to assist with repositioning and lifting.

In addition to working as a Caregiver, the team members serve as a counselor, guide, coach, and mentor in a long-term care setting, building ongoing relationships with the individuals served. Caregivers provide client support through daily activities to improve the client’s quality of life. Responsibilities/Duties:

  • Perform housekeeping tasks (making beds, laundry, washing dishes) and prepare nutritional meals.
  • Assist clients with personal hygiene, medication administration, and household activities.
  • Contribute to a positive living environment while enhancing the client’s quality of life.
  • Report needs or changes pertaining to client wellbeing to leads or management.
  • Complete daily documentation of client activities, goals, and behaviors.
  • Demonstrate high accountability and leadership skills, maintaining professionalism and encouraging positive work relationships.
  • Demonstrate an accurate understanding of company policies/procedures and uphold them consistently.
  • Listen to client concerns, respond in a timely manner, and relay concerns to Program Leads or Managers.
  • Demonstrate sound judgment and handle difficult or stressful situations in a tactful, professional manner.
  • Maintain the ability to work independently without direct supervision and adapt to changing situations.
  • Drive company vehicles to transport clients to work, appointments, and community activities (required for staff with a valid driver’s license).
  • Must be 18 years of age or older.
  • Must be able to lift and carry up to 25lbs independently.
  • Spend time bending, twisting, kneeling, crouching, stooping, crawling, standing, and walking.
  • Use hands and fingers to handle, feel, and operate standard household equipment (vacuum, laundry machine, microwave, oven).
  • Ascend/descend stairs and participate in extended walks as needed.
  • Communicate frequently and clearly with clients to exchange accurate information.
  • Maintain visual awareness to monitor the environment and client safety throughout the shift.
  • Perform personal care and hygiene assistance based on the specific needs of the client.
  • Ability to be outside for extended periods in various weather conditions.
  • Operate within a certified adult family home, utilizing standard household and basic office equipment.
  • Use cellular phones and technology as required, while prioritizing client interaction.
  • Participate in mandatory Crisis Prevention Intervention (CPI) training.
  • Assist in managing behavioral outbursts by implementing CPI techniques with other staff members when necessary.

Pay: $17.50 - $20.00 per hour
