Case Management Administrative Coordinator
Full-Time | Monday–Friday with Possible Weekend Coverage
Are you an experienced healthcare professional looking to transition into a growing administrative role? We are seeking a Case Management Administrative Coordinator to join our team and support patient care coordination across hospitals, skilled nursing facilities, physicians, and health plans.
This is a great opportunity for someone with experience as a Medical Assistant (MA), referral coordinator, medical office specialist, authorization coordinator, or healthcare administrative professional who understands medical terminology, insurance processes, and patient care workflows.
About the Role
The Case Management Administrative Coordinator provides essential support to our case management operations team by helping coordinate patient transitions, manage healthcare documentation, process authorizations, and communicate with providers and facilities.
The ideal candidate is organized, detail-oriented, comfortable using multiple computer systems, and enjoys working behind the scenes to support patient care.
Responsibilities Include:
· Assist with hospital and skilled nursing facility (SNF) coordination
· Track patient admissions, discharges, and care transitions
· Process and manage inpatient, SNF, and transportation authorizations
· Review and maintain healthcare documentation
· Communicate with hospitals, physicians, medical offices, and facilities
· Support case managers with daily operational needs
· Update patient information in healthcare systems and electronic medical records
· Maintain accurate reports, logs, and documentation
· Assist with additional healthcare administrative projects as needed
Preferred Qualifications:
· Previous healthcare experience preferred (Medical Assistant, referral coordinator, authorization coordinator, medical office, or similar role)
· Understanding of medical terminology
· Knowledge of insurance plans, authorizations, referrals, or managed care preferred
· Experience working with EMR/EHR systems preferred
· Strong computer skills, including Microsoft Office
· Ability to learn new healthcare software systems
· Excellent communication and customer service skills
· Strong attention to detail and ability to multitask
