What You’ll Do (Your Day-to-Day Impact)

As a Visiting PT, you are the cornerstone of physical restoration and patient mobility. You will:

• Deliver Personalized Care: Perform meaningful home visits to clients across Hudson or Essex County, transforming their living space into a center of rehabilitation.
• Assess & Innovate: Perform diagnostic tests and physical measurements to develop and implement highly individualized care plans.
• Monitor & Adapt: Perform continual assessments of patient progress and update plans of care to match their evolving needs.
• Educate & Empower: Provide vital education and hands-on instruction to patients, their caretakers, and family members to ensure safe, long-term recovery.
• Collaborate Dynamically: Coordinate seamlessly with other members of the patient's care team and accurately document observations, interventions, and evaluations in the patient's electronic chart.

What You Bring to the Table

• Credentials: A current, valid New Jersey Physical Therapist (PT) License.
• Next-Gen Welcomed: We gladly welcome new licensees! We provide the paid training and support needed to launch a successful career in home health.
• On-the-Road Readiness: A valid driver's license and reliable transportation to navigate your territory.
• Tech Adaptability: Ability to learn and navigate electronic documentation systems (such as Homecare Homebase or others).
• Soft Skills: Strong interpersonal and customer service skills with a genuine sense of empathy.
• Bonus Points: Bilingual skills or additional training certificates are a major plus!
