Duties
- Communicate with families and residents
- Provide supervision to staff performing ADL's.
- Direct communication with the MD if needed.
- Follow up and document on any incidents/accidents.
- Perform comprehensive patient assessments, including vital signs, physical examinations and health histories to form care plans.
- Administer medications and injections accurately following prescribed protocols, including IV infusions and medication administration.
- Document all patient interactions meticulously within Electronic Health Record (EHR) PointClickCare, ensuring compliance.
- Collaborate with interdisciplinary teams on discharge planning, case management and post-acute care strategies for diverse patient populations.
- Provide education on health topics such as nutrition, medication management or disease-specific care plans like Alzheimer's or hospice care.
Requires an Active and Unrestricted LPN License
