Job Overview

We are seeking an experienced Nurse Practitioner (NP) licensed in Arizona to support our Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs. In this role, you will deliver high-quality, virtual care to patients with chronic conditions, leveraging technology and data insights to proactively manage health outcomes.

You will oversee a panel of patients enrolled in RPM and CCM services, using real-time and asynchronous health data (e.g., blood pressure, glucose, weight) to identify trends, intervene early, and coordinate care. Working within our Connected Care Program, you will collaborate with a multidisciplinary team to ensure patients receive comprehensive, continuous, and patient-centered care.

This role requires strong clinical judgment, comfort with telehealth and remote monitoring tools, and a proactive, data-driven approach to chronic disease management.

Key Responsibilities

Remote Patient Monitoring (RPM):

• Review and interpret patient-generated health data (e.g., blood pressure, glucose levels, pulse oximetry, weight).
• Identify abnormal trends, escalate concerns, and intervene in a timely manner.
• Adjust care plans based on RPM data in collaboration with supervising physicians.

Chronic Care Management (CCM):

• Manage a panel of patients with chronic conditions such as hypertension, diabetes, COPD, and heart disease.
• Develop and maintain individualized, evidence-based care plans.
• Conduct routine telehealth visits and monthly CCM check-ins to ensure continuity of care.

Virtual Care Delivery:

• Conduct telehealth visits via video and phone for ongoing chronic disease management and acute concerns.
• Assess, diagnose, and treat patients within scope of practice, ensuring high-quality, patient-centered care.

Care Coordination & Collaboration:

• Partner with care coordinators, physicians, and other healthcare professionals to deliver integrated care.
• Ensure seamless coordination across care settings, including referrals and follow-ups.

Patient Education & Engagement:

• Educate patients on chronic disease management, medications, and lifestyle modifications.
• Encourage adherence to treatment plans and RPM device usage.
• Communicate complex medical information clearly and compassionately.

Clinical Compliance & Documentation:

• Maintain accurate, timely documentation in the electronic health record (EHR).
• Ensure compliance with CMS guidelines for RPM and CCM billing and documentation.
• Adhere to HIPAA and all applicable federal and Arizona state regulations.

Technology & Data Utilization:

• Utilize telehealth platforms and RPM tools effectively to monitor and manage patient care.
• Leverage data insights to drive clinical decisions and improve patient outcomes.

Operational Accountability:

• Maintain a secure, HIPAA-compliant remote work environment.
• Meet productivity, quality, and patient engagement metrics.
• Adhere to assigned schedules, including occasional extended hours as needed.

Professional Development:

• Stay current with best practices in chronic care management, telehealth, and remote monitoring.
• Participate in training and quality improvement initiatives.

Requirements

Licensure:

• Active, unrestricted Nurse Practitioner (NP) license in the State of Arizona.
• Must be in good standing with the Arizona State Board of Nursing.

Board Certification:

• Current national board certification (e.g., AANP, ANCC).

Experience:

• Minimum 3–5 years of clinical experience as a Nurse Practitioner in primary care, internal medicine, or family medicine.
• Experience managing chronic conditions (e.g., diabetes, hypertension, cardiovascular disease).
• Prior experience with Remote Patient Monitoring (RPM), Chronic Care Management (CCM), or telehealth strongly preferred.

Medicare & Compliance:

• Active Medicare enrollment (or ability to obtain).
• No exclusions from federal or state healthcare programs (OIG/GSA).

Clinical Skills:

• Strong clinical decision-making and ability to manage chronic conditions remotely.
• Experience interpreting patient-generated health data and adjusting care accordingly.

Technical Proficiency:

• Comfortable using telehealth platforms, EHR systems, and RPM devices.
• Ability to troubleshoot basic technical issues with patients.

Communication & Patient Care:

• Excellent communication skills with a patient-centered, empathetic approach.
• Ability to build rapport and engage patients in long-term care plans.

Work Style:

• Highly organized with the ability to manage a patient panel efficiently.
• Comfortable working in a fast-paced, data-driven virtual environment.
