Job Summary:

Responsibilities:
Provide direct patient care to our residents, including administering medications, taking vital signs, and assisting with daily living activities
Assist with wound care, treatment, and rehabilitation services as needed
Collaborate with other healthcare professionals to develop and implement individualized care plans
Monitor and report changes in resident conditions to the healthcare team
Maintain accurate and up-to-date records and reports
Participate in quality improvement initiatives to ensure excellent resident outcomes
Requirements:
Current LPN license in the state of Missouri

Ability to work variable shifts, including days, evenings, and weekends
