Job Summary

The Registered Nurse (RN) is responsible for providing high-quality nursing care to residents in a skilled nursing facility. The RN assesses residents' health status, develops and implements individualized care plans, administers medications and treatments, and collaborates with the interdisciplinary healthcare team to promote the highest level of resident health, safety, and quality of life.

Essential Duties and Responsibilities
  • Perform comprehensive nursing assessments upon admission and on an ongoing basis.

  • Develop, implement, and evaluate individualized resident care plans.

  • Administer medications, IV therapy, injections, and treatments in accordance with physician orders and facility policies.

  • Monitor residents for changes in condition and promptly notify physicians and family members as appropriate.

  • Document all nursing care accurately and timely in the electronic medical record (EMR).

  • Supervise, delegate, and support Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs).

  • Respond to medical emergencies and initiate appropriate interventions.

  • Coordinate care with physicians, therapists, pharmacists, dietitians, social workers, and other members of the interdisciplinary team.

  • Ensure compliance with state and federal regulations, infection prevention standards, and facility policies.

  • Participate in quality assurance and performance improvement initiatives.

  • Educate residents and families regarding diagnoses, medications, treatments, and discharge planning.

  • Promote resident rights, dignity, and person-centered care.

  • Maintain confidentiality of resident information in accordance with HIPAA regulations.

Qualifications
  • Current Registered Nurse (RN) license in good standing.

  • Graduate of an accredited nursing program.

  • Current CPR/BLS certification.

  • Previous experience in skilled nursing, long-term care, rehabilitation, or acute care preferred.

  • Knowledge of Medicare, Medicaid, and long-term care regulations is preferred.

  • Experience with electronic health records (EHR/EMR) preferred.

  • Ability to prioritize multiple tasks in a fast-paced environment.

  • Leadership skills with the ability to supervise and mentor nursing staff.

  • Strong organizational and documentation skills.

  • Ability to work independently and as part of an interdisciplinary team.

Physical Requirements
  • Ability to stand, walk, bend, lift, push, and pull throughout the workday.

  • Ability to lift or assist with lifting up to 50 pounds, with assistance when appropriate.

  • Ability to respond quickly to resident emergencies.

  • Frequent reaching, stooping, kneeling, and transferring residents using proper body mechanics and equipment.

Work Environment
  • Skilled nursing and rehabilitation facility.

  • May work days, evenings, nights, weekends, holidays, and on-call shifts as assigned.

  • Exposure to infectious diseases and bodily fluids; appropriate personal protective equipment (PPE) is provided and required.
